Filing Requirements
Once your documents are prepared, you must bring the following to the Circuit Court Clerk’s Office:
- Completed Name Change Documents
- Certified Copy of your Birth Certificate
- Proper Identification (Driver’s License or State ID)
Fees & Costs
A non-refundable filing fee is required at the time of submission:
$175.00
Check with the Clerk’s office for accepted payment methods (Cash, Credit, or Money Order).
Important: Documents must be filed in the Circuit Court Clerk’s Office within the county where you currently reside. Ensure all forms are legible and the birth certificate provided is a certified original, not a photocopy.
